    Notifiable Diseases

    A current list of the conditions and diseases that are required to be reported can be found on the Texas Department of State Health Services website. Both confirmed and suspected cases should be reported, as well as any outbreaks or unusual group expressions of disease. When reporting, please include the patient's name, age, sex, race/ethnicity, DOB, address, telephone number, disease, date of onset, method of diagnosis, and name and contact information of the physician.


    Timely reporting helps us to monitor county health trends, identify potential disease outbreaks, prevent secondary transmission, and evaluate the effectiveness of control measures.


    To report immediately notifiable diseases during non-business hours call 512-216-0162. If no answer, leave a message and your call will be returned within 24 hours. 


    Tuberculosis (TB)

    • To report or refer a patient for TB testing and/or treatment, please complete this form and fax it to 512-393-5530
    • Call 515-393-5569 to speak to the TB case manager

    • How To Report a Notifiable Disease

      To report a notifiable disease (suspected or confirmed), or a public health emergency:


      Call 512-393-5520 to speak to an epidemiologist; or


      Submit a Notifiable Disease Report Form;

      • Fax to HCHD at 512-393-5530; or
      • Email the form through a secure email address to HCHDInfectiousDisease@co.hays.tx.us;

To help keep our community safe, outbreaks or any unusual occurrence of a condition/disease should be reported, even if it is not on the List of Notifiable Conditions.
